GEELONG NINJA TRAINING RULES, ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT
CHILD ADDENDUM & ASSUMPTION OF RISK
Geelong Ninjas workouts pose a significant risk of serious injury to children and all participants. Obstacle and Ninja training involves inherent hazards; risks include, but are not limited to, cuts, bruises, broken bones, slips, trips, falls, collisions, and sprains.
Our Ninja classes are structured sessions with coach supervision, and safety is our primary focus. Management and staff take every precaution to prevent accidents and injuries. If a child acts in a way that poses a risk to themselves, staff, or others, they will be removed from the program. In the event of an accident or injury, our staff will contact emergency services (Ambulance) without delay. A qualified First Aid Officer is always on the premises, and we maintain an automated external defibrillator (AED) onsite.
Geelong Ninjas does not provide supervision for children outside of their scheduled class times. While inside the facility, you agree to take full responsibility for monitoring your child/ren to ensure they follow all rules and regulations.
WAIVER OF LIABILITY
I agree to release and discharge Geelong Ninjas (Geelong Ninjas Pty Ltd), its personnel, owners, operators, facility owners, agents, volunteers, and all related parties ("Released Parties") from all liability whatsoever. I waive all claims, demands, and actions for any injuries, harm, or damages sustained by the child/ren named below in connection with their use of, or presence at, our location or any sponsored event (on-site or off-site).
This waiver applies to all incidents arising from negligent acts or omissions by myself, any other child at the facility, members, guests, or visitors. I agree to defend, indemnify, and hold Geelong Ninjas and the Released Parties harmless against all claims—including attorney fees and costs—brought by anyone related to such injury, harm, or damages.
SPONSORING ADULT RESPONSIBILITIES
By signing below, I agree to be the responsible sponsoring adult. I accept full responsibility for the supervision of the named child/ren to ensure they follow all facility rules while at Geelong Ninjas or a sponsored event.
In the event of an accident or incident requiring medical attention, I agree that as the responsible sponsoring adult, I am responsible for all decisions related to medical care, life-sustaining procedures, medication, and transportation to a medical facility. I accept all associated costs for such care and transportation. I warrant that the child/ren are in good physical condition and do not have any medical conditions, injuries, or medications that would preclude them from safe physical activity.
NON-PARTICIPATING CHILDREN
I understand that Geelong Ninjas is not responsible for children who are not participating in a structured class. If I bring a non-participating child into the facility, they must remain away from all equipment and obstacles at all times to avoid injury to themselves or others. I agree to supervise all children in my care at all times and take full responsibility for their actions and any damages that may occur.
LEGAL GUARDIANSHIP ACKNOWLEDGMENT
As the true legal guardian of the minor child/ren named below, I hereby request their admission to Geelong Ninjas and agree to all terms stated herein.
